The gift message that gets missed is the one typed into a generic order notes box that fulfilment never opens. Where you can put a proper gift message depends on your plan and on checkout extensibility. Cart page gift message fields, handled with a line item property or cart attribute, are available to almost everyone and print on packing slips if you set them up. Checkout blocks are cleaner but gated.
A cart attribute field labeled for gifting, with a character limit and a checkbox that reveals it, captures the message before checkout and stores it on the order where staff can see it. Add it above the checkout button, not in the footer of the cart page.
Test it with a real order and check three places: the order page in admin, the packing slip, and any fulfilment app you use. A field that only shows in one of those three will fail during the holiday rush.
On plans that support checkout blocks, you can add a gift message field inside checkout itself, which catches the shoppers who use express checkout and skip the cart. That is the real advantage, not the design.
The limitation is honest: if your plan does not support it, no app can legally insert a custom field in the middle of Shopify checkout. Apps that promise it are using the cart or the post purchase page instead.
Map the message to the packing slip template and to your warehouse or 3PL export. Write the instruction down for seasonal staff: gift orders get the card, no price paperwork in the box.
Add a tag or a flow that flags orders containing a gift message. During peak weeks, a filtered view of gift orders prevents the normal batch picking routine from burying them.
Character limits matter. Long messages overflow printed cards, so set a limit around 200 characters and tell the shopper. Emoji in messages can break some packing slip printers, so strip or replace them in the template.
Gift message plus gift wrap are separate requests. If you sell wrap as a product or cart add on, keep the two fields distinct on the slip or staff will wrap without the card.
Do not forget the confirmation email. If the gift message appears there, the buyer can check the wording and catch a typo before the parcel ships, and support gets one less panicked message during the busiest week of the year. It is a five minute template edit that pays for itself every December without fail.

No. Checkout blocks require a plan that supports checkout extensibility. Other plans should use a cart field or post purchase step.
As a cart attribute on the order in admin. You need to add it to packing slips and fulfilment exports yourself.
A handwritten or printed card is usually free as a conversion aid. Gift wrap is the part stores commonly charge for.
Around 150 to 250 characters fits standard gift cards and keeps packing slips readable.
